Grant Title: Integrated Care for Individuals with HIV/AIDS, Mental Illness, and/or Substance Abuse Problems.
Project Period: 10/94-09/97.
Description: The objective of this project is to develop and to implement an "Integrated Model of Care" for patients with HIV that suffer from mental illness and/or have substance abuse problems. An enhanced case management system (through collaborative efforts with the Department of Mental Health) will be developed which will simplify referral services for mental health and substance abuse treatment services.
