Health Initiatives for Youth Project Summary

Project Evaluation Instruments

Speakers Bureau Training Feedback Questionnaire
This form is for an evaluation of a three-day intensive training on public speaking skills, the process of transforming a personal story into a public testimony, HIV knowledge, and the ability of the trainers. In addition to demographic information on participants, the questionnaire asks for information such as three new things learned in the training, and comfort in leading a speaking engagement after the training.

Speakers Bureau Self-Evaluation [N/A]
The self-evaluation form was developed by the Speakers Bureau Team for completion by its members. It is used to rate the effectiveness of their speaking engagement. The form asks about feelings that came up for the speaker, how they responded to the feelings, the common denominator with this audience, whether the speaker made a connection with the audience, and how well the speaker interacted/related with their speaking partner.

Speakers Bureau HIV Knowledge Test
This is a test given to Speakers Bureau members about their knowledge of HIV disease. This test is administered after completion of the HIV knowledge session as part of the three-day intensive Speakers Bureau training.

Communication Team "Get A Job" Workshop
Evaluation form for a one-day training for HIV-positive youth or at risk youth ages 16-22 designed to increase their employability. Participants use the form to rate the usefulness of the workshop, how well the workshop increased their knowledge of resume writing, the current job market, interviewing skills, and job searching.

Young Women’s Health Team Initial Assessment Questionnaire
Initial assessment questionnaire on health issues unique to young women, 12-18 years of age in the Bay Area, administered at the start of 10-week work groups. The purpose of the work groups is to discuss health issues unique to women in a supportive environment. Information asked for includes rating the importance of workshop topics (e.g. learning writing and communication skills, learning about drugs and alcohol, learning about STDs or HIV/AIDS, etc.), HIV knowledge, job skills knowledge, self-esteem, and utilization of medical and mental health services.

Young Women’s Health Team Improvements and Skills Test Questionnaire
Administered at the end of the 10-week work groups to determine increase in participants’ knowledge around health issues, how well participants learned specific topics, ability to access healthcare, and increases in self-esteem.

Young Women’s Health Team Program Evaluation Form
Anonymous evaluation form administered after each work group session to document what each participant was able to obtain from the work group, including what they liked best and least about the specific session.

Young Women’s Health Team Short Feedback Form
Feedback questionnaire for the Young Women’s Health Team to document daily proceedings in their work group.

National Leaders Internship Program
Evaluation forms administered to the participants of the National Leaders Internship Program, HIV-positive youth ages 19-25 from the various HRSA/HAB's SPNS Adolescent Care Projects, after participation in the trainings on public speaking, public policy, and advocacy.

Unity Jam
Evaluation of a one-day HIV education and advocacy event. Evaluation of the individual workshops is also available. Demographic information, HIV knowledge, and usefulness to participant are rated.

Ryan White Youth Conference
HIFY staff conducted three one-hour workshops on "Coping with Overexposure", "Empowered Youth Evaluation", and "Kewl Tools to Get Your Message Out" for youth under the age of 26 living with HIV and providers.

Reality Response Card
This one-page questionnaire is mailed out with each subscription to HIFY’s Reality magazine for youth to access what the readers think about specific issues, including the writing style, topics discussed, and usefulness to reader.

Project Report

Health Initiatives for Youth: A Model for Youth as a Resource in Program Evaluation
HIFY used an Empowered Youth Evaluation (EYE) Model (developed in collaboration between The Measurement Group and the HIFY-YHI team) in which youth are empowered to help other youth through skill building, shared activities, and effective communication. Evaluation forms from the cross-cutting evaluation of HRSA/HAB's SPNS Adolescent Care Projects were used to document client characteristics and services provided. Project specific evaluation tools were developed in collaboration between The Measurement Group and the HIFY-HYI team and were used to evaluated specific events and activities conducted by the HIFY-YHI team.

Evaluation of Client Retention in Services in 10 Adolescent HIV Programs
This poster, presented at Evaluation '98–the 1998 conference of the American Evaluation Association in Chicago, Illinois, shows patterns of retention in care by the 10 Adolescent Care Projects. In addition to overall retention, the presentation addresses two related questions: 1. When in a treatment episode do young people receive psychosocial counseling and peer support services?, and 2. How does utilization of case management affect other service utilization?

Cross-Cutting Evaluation Papers from the Adolescent HIV/AIDS Care Projects
These papers present cross-cutting evaluation results from the 10 HRSA SPNS Adolescent HIV/AIDS Care Projects. Results presented in these papers include those relating to the characteristics of thousands of young people served through this initiative, the services provided by these 10 innovative care models, and various outcomes related to service utilization patterns, retention, and other program impacts.

The Infrastructure for Adolescent HIV Services in Six Cities: A Cross-Cutting Evaluation of the Health Resources and Services Administration Special Projects of National Significance Program
This report summarizes the findings of adolescent care projects in six cities (San Francisco, Boston, Los Angeles, Bridgeport, Seattle, and Birmingham) in relation to evaluating the service infrastructure for HIV-related youth services. The report looks at the type and quantity of HIV-related service availability for youth in each city as well as the barriers to providing such services. In addition, this report documents the cohesiveness of the service continuum as well as service provider consensus of HIV youth services design.

Youth-To-Youth–Peer Workers in HIV/AIDS Youth Programs: A Peer Development Guide
In 1993, the Health Resources and Services Administration (HRSA) Special Projects of National Significance (SPNS) Program funded ten national demonstration projects focusing on youth and adolescent HIV/AIDS services. Seven of these projects have collaborated to assemble a manual describing the best practices and lessons learned from programs that use peers in the development and delivery of HIV services. Provides a summary, list of editors, and table of contents of the Journal of Adolescent Health Supplement on Ten Models of HIV Care.  The supplement contains a Preface, information about the ten adolescent models of HIV care, a national consensus model for adolescent-targeted services, and articles about the HIV care models.

Report on Outreach Activities in 10 Adolescent-Targeted Projects for HIV/AIDS Services
A series of reports are being written about the activities of the 10 projects on adolescent-targeted HIV services funded in 1993 by the HRSA Special Projects of National Significance (SPNS) Program. This first report examines the methods that the projects use to recruit adolescents with, or at high risk to acquire, HIV. The report was written by The Measurement Group in collaboration with the Adolescent Care Projects and HRSA staff. The report covers the period from 1993 to 1996.

Report on Enrolled Clients in 10 Adolescent-Targeted Projects for HIV/AIDS Services
