The Special Projects of National Significance HIV Innovative Models of
Care Program is an effort by 27 cooperative agreements funded by the Health Resources and
Services Administration (HRSA) to jointly establish goals and objectives, develop common
evaluation methods, and produce comparable and measurable outcomes. The Evaluation and
Dissemination Center for HRSA/HAB's SPNS Cooperative Agreement, in collaboration with the
grantees and HRSA, has designed and implemented a core evaluation to capture common
elements across the 27 SPNS Program projects. The cross-cutting or national evaluation for
the cooperative agreement projects uses a modular approach in which standardized forms
were developed to answer a number of different functional evaluation questions. Modules
include short 1-2 page surveys, self-administered questionnaires, checklists, interview
forms, and other standardized data collection protocols. Many of the evaluation modules
were developed in a scannable form that can either be faxed for automatic entry into the
database using an innovative fax-in data transfer technology system, or mailed in for
scanning. Data forms, received either from the fax or from the scanner, are automatically
appended to the project database thus yielding data which is immediately up-to-date. By
developing very brief modules, data collection efforts for different projects are
customized by including those modules that fit the purpose of the project. By using common
modules across the 27 projects, as much comparable data as possible may be obtained. The
