Missouri Department of Health lntegrated Care Program

The Missouri Integrated Care Program is one of 27 Special Projects of National Significance (SPNS) Program demonstration projects across the country. The Missouri SPNS project has an evaluation and training focus and is designed to offer integrated services for poly-diagnosed individuals living with HIV/AIDS (HIV-positive, mental illness and/or substance abuse issues) in Missouri. A Memorandum of Agreement, the first of its kind in Missouri, was established between the Missouri Department of Health and the Missouri Department of Mental Health to facilitate the development and operation of the Integrated Care Program. Three sites; Kansas City, St. Louis, and Southeast Rural Missouri were chosen for the development of integrated models of care involving integration of HIV case management with mental health services. Advisory committees were formed to develop the integrated models of care according to the needs and nuances of each site and assist in the planning of site specific trainings. In this poster presentation: the goals of the Missouri Integrated Care Program are discussed; the three models are examined; and the evaluation of the program and the three sites is explored.
