University of Nevada Project: Client Telephone Interview
The following telephone interview template was developed by The Measurement Group and is used by the University of Nevada School of Medicine to gather information about the experience and services received by clients of their HIV nutrition clinic. This interview is presented as a sample of a client feedback interview to give information about an HIV clinic.

Permission is granted for non-commercial use so long as form is not altered, the copyright is not removed, and a proper citation is made to the instrument.
