Main Purpose	The purpose of this module is to have a standardized way of asking about the HIV testing history of the individual. The questions also ask about what happened after the individual was tested.
Description	Items for this form were developed for and used in the HRSA study of HIV-infected drug abusers. Also these items were adapted for use in the 11 Adolescent SPNS Projects. Administered in 3-5 minutes.
Target Projects for Form	Any project documenting HIV testing history for individual clients.
Major Evaluation Function	Characterize history and experiences of project clients. Use history and experiences to predict treatment compliance, outcomes, and retention in treatment protocols.
