Main Purpose	The purpose of this module is to record major episodes of technical assistance for the purposes of agency development, outreach, fund-raising, grant writing, organizational development and management.
Description	The form codes materials provided, impact of technical assistance, and method used. A codebook for writing out data for this module from alternative databases has been developed. The actual or approximate number of individuals impacted by major demographic categories. These data are collected at the aggregate level. Additionally the form codes the purpose of the activity, the setting, and major topics discussed.
Target Projects for Form	Any project that conducts technical assistance, including projects that provide written materials, consultation on evaluation, assistance with computer software or hardware, etc.
Major Evaluation Function	To record technical assistance performed by the project and the number of people upon which the activity potentially had an impact.
