Content Domain - Ratings of overall progress including changes.

Operational Data Items - Site, Sub-Provider, Date, Staff Code, ID Letters/ID Numbers, Karnofsky Scale, Rating of Patient/Client Adherence to Treatment, Barriers to Adherence, Level of Patient/Client Satisfaction with Progress.
