Module 45: Life Stressors Form

A Module Used in The Measurement Group's Cross-Cutting Evaluation of the HRSA HIV/AIDS Bureau's Special Projects of National Significance on Innovative Models of HIV/AIDS Care


Purpose

  • Standard measure of the presence of significant sources of life stress.

Content Domain - Checklist of major life stressors and valence associated with each.

Operational Data Items - Site, Sub-Provider, Date, Staff Code, ID Letters/ID Numbers, a list of events that the client may or may not have experienced, ratings of the severity of each of the events.
